If you are interested in being considered as a guest on Michigan Entrepreneur,
please fill out the application. Completing and returning the application
is not a guarantee of a guest appearance on the program. Completed
applications should be mailed to contactmetv "at"
michiganentrepreneurtv "dot" com.
Dub Request Form
Dub Request Form
If you would like to order a copy of your program, you can fill out a dub
request form and remit the proper payment to BCTV. The studio has many
different formats available including VHS and various DVD formats.
Standard Release Form
Standard Release Form
All guests are required to complete a Standard Request form in order to appear
on Michigan Entrepreneur Television. This form gives Bloomfield Community
Television the right to image your likeness and to record your voice. It
is a good idea to review this form before you arrive at the studio. I will
provide you a copy to fill out when you arrive at the studio.

Bloomfield Community Television is located just south of Long Lake and West
of Telegraph road, right between Andover High School and Telegraph. It is
a single story, red brick building. Please download a copy of the map to
the studio.
