Standard Release Form
All guests are required to complete a Standard Request form in order to appear on Michigan Entrepreneur Television. This form gives Bloomfield Community Television the right to image your likeness and to record your voice. It is a good idea to review this form before you arrive at the studio. We will provide you a copy to fill out when you arrive at the studio.
